Clinical decision support systems (CDSS) have long promised to reduce diagnostic delays and improve guideline adherence, yet evidence of real-world impact remains limited. MAIA (Medical Artificial Intelligence Assistant) is a hybrid CDSS developed by Tiplu GmbH and CE-marked as an MDR Class IIa medical device. It combines rule-based alerts for presumed diagnoses with machine learning models for risk predictions. Both operate on continuously updated patient data (including laboratory values, vital parameters, and clinical documents) retrieved via standardized FHIR interfaces from a clinical data repository connected to the hospital information system and subsystems. The underlying clinical prediction models were trained by federated learning on anonymized inpatient records from approximately 130 German hospitals. This talk outlines MAIA's development and core functionalities, the real-world advantages of an interoperable FHIR-based data source, preliminary real-world validation results regarding sepsis prediction, and future perspectives on bringing LLM-based decision support into routine clinical care.

Dr. med. Julian Hugo, M.Sc., is Medical Lead Clinical Decision Support at Tiplu, where he heads a team of physicians supporting the development of AI-based clinical decision support systems. He studied medicine at the Julius-Maximilians-University Würzburg and earned his doctorate in neurology with laboratory research on the excitability of nociceptive neurons. Alongside two years of residency training in clinical pharmacology, he completed an M.Sc. in Digital Health at the Hasso Plattner Institute in Potsdam. His master's thesis on machine learning-based risk prediction in Crohn's disease received the Mario Stefanelli Award for Best Student Paper at AIME, the GMDS Förderpreis for the best master's thesis, and the DMEA Nachwuchspreis. Since 2023, he has been with Tiplu, where his roles have included Product Owner of an ML-based clinical decision support product and Venture Owner responsible for developing a clinical LLM platform. His work focuses on data interoperability, AI-based clinical decision support, and the application of large language models in healthcare.
